1. Your valid signed passport (not a copy)
2. Application form fully completed and signed
3. 2 Passport-type photographs
4. Invitation from Host Company or Organization in Azerbaijan, approval which is received by the Embassy, from the Ministry of Foreign Affairs of Azerbaijan, or a tourist voucher from hotel or tour operator in Azerbaijan that lists the hotel confirmation number or tour reference number.
